Insurance companies use the term Total Permanent Disability. What is the definition of this term and what are the parameters for assessing such condition.

It depends on the insurance company and more specifically on the specific contract of insurance.
For example, ICBC's definition of totally disabled is in the Insurance (Vehicle) Regulation and states and their definition applies to "an insured who is an employed person from engaging in employment or an occupation for which the insured is reasonably suited by education, training or experience"
Other policies have other specific definitions and exclusions. You should look at the language of the policy in question as well as the exclusions. Tuypically words such as "permanent" "total" and "disability" are specifically defined in the contract of insurance.
